As of the fourth quarter of 2023, The Foley Center at Chestnut Ridge was owned by Liberty Healthcare Group, LLC, according to Centers for Medicare and Medicaid Services (CMS). The nursing home, located in Watauga County, has been owned by Liberty Healthcare Group, LLC since Oct. 1, 2019.

But when you need that type of care, there are limited options.”
